What evidence is necessary to apply
Social Security needs medical evidence that specifies the type, extent and site of the primary, recurrent or metastatic lesion. When the primary site cannot be identified, they use evidence documenting the site(s) of metastasis to evaluate deterioration. For operative procedures, including a biopsy or needle aspiration, they generally require a resolution note and pathology report.
When they cannot obtain these documents, they accept the summary of hospitalizations or other medical reports. This evidence should include details of the findings at surgery and, where appropriate, pathological findings. In some situations, they may also need evidence about cancer recurrence, persistence or progression, response to therapy, and any significant residuals.
